Individual Disaster Assistance:
When disaster strikes, IMPACT is on the front lines assisting families. Households MAY receive up to $5,000 for qualifying household items. We deliver this service when Iowa’s governor declares one of our service counties a disaster area.
Governor Reynolds has issued a Disaster Declaration for Boone, Jasper and Polk counties.
Families have until September 25, 2020 to apply for assistance.
This application will have to be printed and completed by hand and then returned to IMPACT either via email at email@example.com or mail to IMPACT Community Action Partnership, 3226 University Ave, Des Moines, IA 50311
If you are unable to print an application, you can have one mailed to you. Please request one by sending your name, address and phone number to firstname.lastname@example.org
If you are unable to complete the application on your own, please call 515-518-4770 to be scheduled for an appointment to complete your application

Iowa Disaster Case Management:
Disaster Case Management is a time-limited resource and process that involves a partnership between an IMPACT case manager and a household impacted by a disaster. Through this partnership, the parties work together to develop and carry out a disaster recovery plan. This partnership provides households with a single point of contact to facilitate access to a broad range of resources and promote sustainable assistance.
