Documentation Needed For Utility Assistance

Having the correct paperwork when you apply will make your application easier. If you have any questions about what documentation you will need to submit, our staff are happy to help during your phone appointment. 

• Proof of Social Security Numbers for all household members. Adults can use a valid Iowa Driver’s License instead of a Social Security Card. (More options can be discussed during your appointment if needed.)

• Most recent heating bill

• Most recent electric bill

• Proof of income for all household members.  You will need to provide income from the past 30 days prior to the application date OR for the past year.  The chart below describes the most common income types and what documentation is needed. If you do not see your income type listed, our staff can provide more guidance during your appointment.

• If your family has had no income for the past year, you will need to download, print and sign a zero-income form.
