Documentation Needed For Utility Assistance

You will need the following documents to apply for Energy Assistance through the LIHEAP program:

Social Security Cards for all household members:

• If SS Cards are Unavailable: Tax Returns, W-2s, Unexpired Iowa Driver’s License/Real IDs, U.S. Passports or Birth Certificates, or I-94s PLUS a verbal confirmation of your Social Security number

  • Proof of at least one Social Security Number is required per household – limited SSNs may impact your eligibility & award amount

Income Documentation for all household members 18+ from either…

• The most recent 30 days

  • Pay stubs, 2025 SSI/SSDI Letter, Unemployment Insurance, Pension/Retirement/VA Benefits, etc.

OR 

• Annual Income

  • A 2025 or 2026 Federal Tax return (all pages) is allowable if there is only one adult in the home with income, but we need to verify individual income via W2s/1099s for each person if there are multiple adults with income

OR

• Sign a “No Income Form” at the time of your appointment

Note: the time frame must be the same for all household members

Most Recent Utility Bills for Heat AND Electric Vendors (these may be the same vendor)

• Must have the correct address, account number, and date visible​
